Employees who require reasonable accommodation:

Step 1 - Submit a Request for Reasonable Accommodation Form to Human Resources

Step 2 - HR will request that you provide a Supplementary Medical Questionnaire* and a Job Description to your Medical Provider (*Sample Letter and Questionnaire)

Step 3 - HR will schedule an Interactive Process meeting
